If you are a resident and have a maintenance or facility issue that needs to be addressed, please complete this form. Please make sure to complete every field and provide as much detail about your request as possible. If you have any questions about completing this form, please contact the Office of Residence Life during office hours (M-F 8:30am-12, 1-4:30), or the Resident Manager on duty during non-office hours (M-F 4:30pm-8:30am and 24 hours Saturdays, Sundays, and holidays).
If your request is urgent (such as a plumbing leak), please call the Res. Life Office at (202) 885-8612 during office hours (M-F 8:30am-12, 1-4:30), or call the Resident Manager immediately at (202) 246-8251 during non-office hours (M-F 4:30pm-8:30am, weekends or holidays) .
